Book an Appointment

- Please be sure to select whether you would like a telehealth or in-person visit when booking.
- If you would like to become a member, you will select "become a member" on the consent form included in your intake paperwork, which you will receive after booking a new client visit.
- All intake paperwork for new clients must be completed 2 business days before your scheduled visit. If it is not, your appointment will be rescheduled.
- We currently do not take insurance.
- We accept cash, credit/debit cards for payments. We do not accept checks.
- If you are booking for a minor, we request that you create the account using their name and DOB. You may use the parent or guardian's email address and/or phone number for the account.
